| This screen, from the C-CAP computer program, shows how the assessment process is divided into various modules. This allows the examiner to select the specific module that is relevant. Modules can be completed in any order and a final checklist allows an overview of what has or has not been completed. Screens are formatted for 15" laptop monitors. |

| This shows the first screen of the Interview of Child Module. It shows how answers are recorded both in a checklist format and complete answer format (textboxes). Across the top, note the multiple pages and divisions within this module. Navigation buttons are marked "previous" arrow and "next" arrow. User may also navigate by selecting a page tab located across the top of the screen. Some fields are automatically imported, such as name of child, date and time. |

Summary
Score, Rating Guide and Recommendations
for School History Module |
| This screenshot demonstrates three important features of the C-CAP: Summary Scores, Rating Guides, and Recommendations. The examiner selects a 0 to 3 summary score rating to finalize most modules. Zero always indicated no problems found and three always indicates severe difficulties in a particular area. Rating Guides are pull down windows that assist the evaluator in making Summary Score decisions. Once the Summary Score is selected and the module is completed, it is locked and the Recommendations associated with the selected Summary Score are displayed. The examiner then selects or deselects any recommendation listed. Both Summary Scores and all selected recommendations are automatically transferred to the final report. |

Child
Summary from Final Report

| This screen demonstrates how Summary Scores are displayed in the Final Report. In addition to the Child Summary, there is a Family Summary. This graphic is automatically imported in the Final Report along with all the data collected within the modules and the Recommendations selected. The evaluator provides final diagnosis and may edit any portion of the Final Report. Final Report templates are compatible with various word processor formats. |
